I've got a 2000 X 6.8 that is just being used around the house. I keep it up so it's not deemed "sitting" BUT it's not seeing more than 100 miles a month either. The 1st start has been producing some strange issue. The engine will start, fall off, rev up, then die. Restart is fine & any other starts that day are fine. Truck runs like a top otherwise. This issue is getting progressively worse SO I'm trying to get ahead of it. ANY IDEAS ??? Thanks
Probably fuel pressure leak-down. Cycle the key from OFF to RUN, pause two seconds, then OFF again. Repeat once more and then see if the engine starts reliably.

Will Do... Thanks for the tip. If this corrects the issue is there anything to worry about or is this just an issue of a 16 Y/O non-daily driver ??

Fuel pump, most likely. Not injectors. If it were injectors, you'd have a much bigger issue (potential hydro-lock, fuel-diluted oil, etc).
The FP has an internal check valve. It's probably worn out.
It's not an IAC or vacuum issue if the cycling resolved the symptom. You could install a fuel pressure gauge and actually see what's the pressure in the system.
