Main vac to one, then one to the vacuum reserve on the pass side wheel well (usually looks like a black ball. The last one to the vacuum lines that enter the cab on the pass side by the air box.
So am i suppose to feel any type of suction from these 3 ports because I don't feel anything? I have the Line running from the black ball to the left port but have the other 2 plugged because I don't have anything to plug them into. Truck use to be a 5.0 EFI & coverted it to Carb.
Sorry, the one all the way towards the pass side on that manifold should be capped. main vac to the one on the right. Then hook up the line that leads to the splitter to the center. The splitter will lead one line to the vacuum reserve and the other to the line that enters the cab. Yes, you will feel suctions once you have a main vac line hooked up the the one on the right of the manifold.
I'm getting lost with the descriptions, like the "right one". But, I think bcamill is correct if I understand what is being said. The "splitter" is actually both a tee and a check valve. The large line in the center plugs into manifold vacuum, which can come from one of two places. Most engines have a vacuum tree screwed into the rear of the intake manifold, and the large line can connect there. However, there is usually a small plastic manifold on the firewall, and it has one connection to that tree and another connection can be used for the HVAC controls.
That plastic tee with 3 ports is there to supply vacuum to the HVAC system.
The first thing you need to do is to run a vacuum hose from the center port of that tee to an port on the vacuum tree that is screwed into the back of your intake manifold. If you don't see it, follow the hose from the brake booster.

The open hose needs to be connected to the left port on the 3 port tee in the first picture. That will supply manifold vacuum to power the HVAC system.
