PLEASE HELP! New, different knocking 5.4 3v

My '04 5.4L 3v (201,000+ mi) sounds just as you describe, and always has. At a drive thru window you'd swear it is a diesel engine. About 1100-1200 it quiets down and purrs like a kitten. VTC's were both replaced about 120k, but it made NO difference. 10w30 will only slightly help when cold - but not at operating temp.
Mine also has the 'tick' at idle. More prominent on bank 1. Sounds exactly like one loose solid lifter used to on my highschool hotrod 312cu V8. I do not know what it is, but I (as you said) I don't believe it is an exhaust manifold leak. But it has NEVER gotten worse, so don't worry about it.
Hope you get the same good service out of your 5.4 as I have.
