Bad alt./battery?

Bad alt./battery?
I was driving the other day and my motor started bogging down almost dying. I noticed my volt gauge was bottomed out at 10v or so and figured that was the cause. Replaced the alt. got a jump and it all seemed to be good at 14v then it jumped to 16v then back to 10-11v and started bogging. Replaced the battery today and same thing. Still reading no more than 11v.
What could cause this? Fried the new alt. with a bad battery? Is there anything in the charging system that could have failed that I didn't notice? Thanks!
