Injector Failure
What and where could this thread material come from?
Dealer Shop towel at Install?
Injectors were new remaned and a new heads replaced, run fine for almost a year.
balance test showed #5 at 40% so I took them all out on the Pass Side. This was the worst all the others looked fine.

Where did you buy your reman. injectors at? Did you use Ford Reman.? Auto Nation Injectors?

Warren Diesel, they were checked and balanced and known good.
I just don't know where thread would come from or how it would get on the injector unless it was a shop cloth by the mechanic cleaning the injector cups.
#5 is hard to see into -against the AC Condenser without a mirror, so it could happen, but stay in for a year? ? ?

Can't answer your question since I am going to work on my injectors soon too, my first time.
From what you describe, look like the mechanic did not do a good job on cleaning...

Since I placed all 4 of them from that bank I'll have to buy 4.
Where should I go to get them other than the Stealer...

Warren is rebuilding the full set and is only charging me the labor so i'm happy since it's clearly not their fault and most likely a botched install.

It's hard to tell from the pictures, but is it slivers of o-ring?

No, the O-ring was still intact but brittle, it was fabric thread or some noncombustible fibers wrapped completely around the injector.
