Here's my take on what's happening: The misfire is causing "un-used" oxygen to pass by the HO2S. The HO2S reports this excess oxygen to the PCM. The PCM trys to "fix" it by increasing the injector on-time on Bank 2. When the PCM drives the fuel trims to > 30% and it still sees "unused" oxygen from the misfires it says "sumptin is not right" and sets the P0174 code (which means it is driving the injectors on that bank way more than it should and it still sees unused oxygen, which is interpreted as "Lean".)
If the OP has been driving this vehicle like this for some time the other two cylinders on that bank are drowning in fuel and have probaly carbon/coked up the DPFE tubes and/or the EGR passages in the intake. If the PCM doesn't see enough difference between the pressure on each side of the DPFE, it sets the P0401 - insufficent EGR flow.

The very first thing to do is check your vacuum lines and elbows as well as the PCV itself. It seems that an awful lot of the lean conditions and mifiring problems are caused by the 90* degree elbow at the intake manifold. A vacuum gage would be a help in this case for diagnosis.
