Question about head resurface

I have a 1999 F150 ext cab 2wd flare side 5.4l that has the infamous oil leak on the right side rear. After putting it off for two years I finally got around (forced myself) to replacing the head gasket.

I have read the Ford tsb on this and it's really unclear as to whether or not I should have the head resurfaced. Since the oil leak is the only issue I have had with the head, I am leaning to not having it checked. Does anyone know of any reason I would need to have it checked?

Another question I have is about the PCV end elbow. Mine of course had rotted and was the reason I finally parked the truck some months back. A friend of mine pulled half of the fitting off ( I couldn't get my hands back there) and dropped it. So what I'm left with is just the half that goes on the intake. I have looked at elbows and have been unable to find one that is big enough to fit the intake side. I am also curious as to whether or not mine is actually a 90, or if it could be straight. Does anyone know if Ford used any straight adapters or if they were all 90's?
