Well.i did not ever think this could happen...the fan came off the water pump and destroyed my radiator....i just did water pump but have 200 miles on it since...im baffled...
I never use locktite also might not have been tight enough upon installation that can cause the fan to loosen up enough then fly off when the motor is shut down at a point
yes but you still have to tighten it down when you install it to keep it from happening because when you shut off the motor the fan hub stops but the clutch and fan keeps spinning also a possibly is a locked fan clutch

FAN CLUTCH NUT TORQUE
48mm fan clutch nut. Tighten to 84-113 lb-ft,
pretty hard to get a torque wrench on it though. You just tighten it a much as you can pull the wrenches.

and that is why i always use locktite.
i saw way too many fans into radiators over the years. but not one with locktite on the threads has ever come off.
