Brake Caliper Issue

Hello Everybody.
My truck is an 89 F250, but is has a Straight Axle from a High Boy in it so I figure this would be the forum to ask this question.
So the Dana 44HD was swapped into my truck almost a year ago already, and I don't think the axle has ever been gone through. It only had about 50k on it when it was pulled from the truck that it came from. (Just to give a little background on it)
So here is the issue that has occurred. The H shaped piece of metal that holds the caliper onto the mounts on the rotor, along with the piece of metal that is supposed to be the spring, has worked its way out from the caliper and fallen out. However, the bolt that is supposed to prevent the H shaped piece from sliding still remains in place.
So Ok, no big deal I figured, pulled the bolt out, pounded another one in along with the spring with a Big FN Hammer, put the bolt back in, drove off. Well that one lasted 3 days and is gone. I can't for the life of me figure out what would cause this, but I was hoping one of you would know. The best guess I have is that maybe a bearing is going bad and is allowing it to shimmy out of its place.
Any help is appreciated!
