I have a '99 F-150 4x4 with 131k miles on it and a few months ago I took it to a local shop for some vibration issues from the rear end. They diagnosed it and told me the u joints have failed (I expected that) and also told me the pinion bearing was going out. I had him replace the u joints and I would deal with the bearing later.
After thinking about it I wasnt sold on the bearing going out, so I took it to a specialty driveline shop where I was told that the bearing was fine just the pinion seal was leaking, I had them replace it.
Now driving it I am hearing noise from the rear end and I cant tell if it is a tire a wheel bearing or something else, I dont know what kind of noise a pinion bearing would make if it really was going out. Any thoughts anyone may have would be appreciated.
About the only thing you can do to make a decision is raise the rear off the ground and spin it low speed in gear and listen to the results. If it sounds rough and/or vibrates, that's all you need to know.
Taken apart for any service is about the same work.
Taking the rear plate off might show a wear issue looking at the ring gear contact path for signs of flat spots and metal blistering because the metal is only hardened to a small depth before wear accelerates once the surface hardening has broken through to soft metal.
At least you can get a feel for backlash.
The spec for an 8.8 is .010 to about 015". Not a lot so you could tell if there is really excessive wear by just moving the ring gear back and forth against the pinion to get the feel for it..
Carrier bearing either side could be rough, as well.
Basicly you cannot tell a whole lot without a check out.
Be prepared to do what you need to do before you take things apart.

Ok so I jacked it up and put it in gear, I didnt hear anything abnormal, maybe a little brake drag. The other thing I noticed is the back door on the passenger side is rusted out below the weatherstrip and I'm wondering if I am hearing road noise through this void.
replacing the pinion seal in your truck requires removal of the U-joint yoke. doing this requires remving the diff. cover, and the ring gear, the spider gear assembly, etc; if there was a mechanical issue in the differential, im sure the driveline shop would have pointed it out to you. however, you may be hearing an axle bearing going out.
