1977: Ford made multiple changes to the Courier: All new sheet metal, bumpers, grille and lamps. SOHC 2.3L I-4 and 7' Styleside bed introduced.
1977/82 Courier that someone stuck F350 badges on. The mirrors are similar to what Ford introduced in 1975 for F100/350's and Econolines, but are aftermarket.
1972/82 Couriers (rebadged Mazda B Series pickup) were available as a Cab & Chassis or as a Styleside pickup.
There were several companies here in LA LA Land that would add dual rear wheels, utility, flat or stake beds, or even the creation you see here.
Many FoMoCo Dealers used Courier stake beds to pick up and deliver parts. Stake beds were common back then, but are rarely seen today.
Sean, where and when did you take this picture? The car parked next to the VW is an Oldsmobile Club Coupe, either a 1949, or early 1950 (split w/shield is the 'give-away').
I sold a 1950 in 1981 after owning it since 1962. Regretted selling it within days, have been looking for it ever since. This might be the same car, as very few were made and are difficult to find today.

How about a beater Pinto 'woodie' station wagon? It's about the same O/A length/width as a Courier, might even have the same I-4, and you won't need to look...hither and dither...for a camper shell.
You don't have to go very far to find one, either. One is parked in a driveway on the east side of Michigan (or Milliken, can't recall which) Avenue, just north of Whittier Blvd.
It's been there for quite a while, dunno what's wrong with it outside of its faded red paint, wood grained fibreglass rails and decals.
If you buy it, you could get yourself some boat davits, suspend it from your Super Duty, take it along with you on trips.
