Hmmmm sounds like you have already covered all the bases. ??

a few questions....

Hoe many miles?
Does it stumble only when cruising around 55 mph, or at all speeds?
The idle issue.... does it just hesitate only at initial acceleration?

Any chance it is two different problems?
Like a vacuum leak (55 mph stumble) and maybe a problem with the idle air controller (hesitation at initial acceleration)?

Also consider, there have been several guys who have had problems with the DPFEGR (or DPF sensor)causing rough running, also consider possible clogged EGR ports inside.

Does CEL ever come on?

If I had to make one guess, it would be in the DPF sensor circuit.

Yep. I'd be looking for a vacuum leak.....

It's possible that the EGR ports are partially clogged, or that the EGR valve itself is sticking, but I would think that if the roughness is bad enough that you're asking for help, the you would have thrown a code and have CEL by now. I've had a PFE sensor go bad, and throw a code, but never experienced any roughness or hesitation from it.

Plugged EGR lines on the other hand.......will cause roughness sorta like Roseanne Barr singing the national anthem.

Way to talk to the guy! Shame on you!

You can look for this. I found a vacuum leak, rubber 90 degree elbow from under the throttle body had a hole in it. I went to Ford to replace it and it came with the plastic line to the back of the passenger side of the motor. There was also a rubber line which went to a fitting at the very back of the motor on the passenger side valve cover area.
I did not remove the intake but it was time consuming to install. Our 99 F 150 improved a bit after this but still stumbles off idle.
Hope it works.

Hey there if anyone has these same symtoms before spending hundreds .. Do yourself a favor and check the PCV line there is on my 99 E150 4.2 right rear PCV line you will see a line going 3 places , PCV into valve cover, then intake in rear of intake above the PCV, also it runs forward to the bottom of the throttle body , when you go around front of the vehicle reach in under the throttle body, Mine had DIME sized hole in the elbow, Its called PCV elbow.. you cannot see it from the front easily You must take the cover off the throttle body, i bought the elbow at autozone 12$ ...
email me for the pics i took . jdmksecurity@gmail.com

i have had the same problems , Start up was strong as soon as idle started it stumbled , put it in gear with brake on would lope lope lope, as soon as accelerate was strong and then smooth , till it got to overdrive i would hear it stumble, then when slowing to stop it would stumble . but when driving from stop was good again but everytime idle it would lope like some big cam was installed..
anyway i hope this helps you guys with same problems... I have a great picture of it .. email me i will send it to you so can see what it looks like...
