cant figure it out, help

my van with 241,000 has treated me right for years but the last couple of mornings , after a 20 min warm-up my first 10 miles or so when i let off the accelorator pedal a can feel a jerking in the drive train also it idles up and down . my comute is about 50 miles of all highway it gets almost like its not happening anymore when the van gets good and warmed up. it seems to happen at low speeds 10 to 40 mph . i looked under the van and i can see trans fluid (not much) on the front of the driveshaft ,right where it comes out of the trans tail. is there a bearing in there that could cause this? this van is my every thing for work please any help . the truck is an 97 7.3 extended supervan E350 with the ambulance package. if i shift into neutral while its happening its a nice smooth ride

It should have some play. I dont think there is but not 100% sure. You'll notice if they are very bad.
