Well I sold my old 1997 Regular cab 4x4 ZF5 with 230K on it in July. In return I bought my 1997 Extended Cab Dually automatic 2x2 with 97K on it. I sold to my buddy, so he offered the regular cab back to me for 3500 bucks.
I really Really miss the zf5, and the dually will be converted if I keep it.
next... I really like how the dually tows my bumper pull race trailer...
I dont like the 2x4.
My plans for the dually are to keep it and convert it to 4x4 with a zf5 (i already have the tranny and transfer case). I would need drive shafts and dana 60 and make brackets. But in the end I would have a 4x4 5 speed extended cab dually.
But I figure I might be able to get the regular cab back (which has worn suspension). Put the dually rear axle and bed on it (the trucks are both white). It has a good clutch and a newer tranny in it ( my buddy put a newer one in that shifts better). The whole front end would eventually be turned into a RSK with a Dana 60. I already have the dually hub adapters for the front axles.
The only down fall to that, is the interior is worn out, the suspension is worn, out, and it has 130K more miles.
then I gotta try and sell the extended cab with 102K on it as a 2wd. but with the lower mileage and the condition the truck would be in other than the bed, would hopefully fund my dana 60 front end.

I say go for the 4x4/ZF-5 conversion on the dually. Get an RSK and a D60 and you'll only need a couple rear leaf spring brackets and you'll have a pretty unique truck.

would I need front brakets too? Not too sure, never looked at the 2wd versus the 4x4 front suspension setusp

The only way to answer this question is this.. Would you rather have an extended cab or a regular cab?? Do the miles really matter to you? This is what I would do..
Because I like regular cab trucks, especially regular cab 4x4 duallys, I would take the dually bed and rear end and stuff it under the regular cab and then put the engine from the extended cab in the regular cab. Then switch the engine and stuff from the extended cab to the regular cab so then you will have a lower mileage engine and a non cali truck and put in a PCM from a federal emissions manual trans truck.
Sell the extended cab as a cali emissions 2wd single wheel and call it done.
If you would rather have the extended cab configuration though do all the work to that one and then you don't have to switch over to federal emissions and you just gotta do the front axle and transmission swap.

RSK won't work with the D50 TTB. Another option (even though it's pricey) would be 05-07 dually axles. Since the 2wd has coils up front, it might be a more straight forward swap than the guys converting leaf sprung OBS 4x4's into coil sprung trucks. It takes a bit of fab work, but the guys that have done it love it. Not sure if that's up your alley or not, but something to consider.
travis said it best. what cab do you want? you said you like the dually and want 4wd.... sounds to me thats the way to go. put all the best goodies in the dually. just an opinion.. single cab duallys do look sweet!
Seems like an awful lot of work. What transfer case do you have? What yoke is on the output shaft to front end? If it were me I'd be looking for 4X4 dually in what ever cab I chose. Probably would be cheaper in the ling run. Hate to see a good truck turn into a parts truck. There is a guy in Welington Nevada that knows all the ins and outs of conversions.
I would just make the extended cab a zf5/4x4. I think it will be less work in the end. Keep the truck you have and make it what you want, rather than going back to the old truck and swapping all the duallys stuff onto it.
