The spool just replaces the differential, not the ring or pinion.
You have to unbolt the ring gear from the differential and replace the differential with the spool and bolt the ring gear back on.

spools don't but the gears do. You will have a 'case break' at certain gear ratios, meaning that a particular case can only handle up to, say 4:10 gears, and after that you will need a new case because the gears are physically bigger.
