P0156 & P0161 O2 Sensor

P0156 & P0161 O2 Sensor

I hope I'm not overthinking this... I got those two codes which both point to the same O2 Sensor (Bank 2 sensor 2). At first glance, I might be tempted to replace the sensor in question, but I've seen a lot of other things trigger O2 codes. Should I replace the sensor? Which is bank 1 and which is bank 2?
Truck is 97 expedition with 170K and a 5.4L engine.

FYI, bank 1 is always the side with the #1 cylinder, which in our case is the passenger side. For you, bank 2 sensor 2 is the O2 sensor on the driver's side after the catalytic converter.
