Transmission slipping? Slip yoke or just yoke bad?

I'm trying to diagnose a problem with my truck from 7000 miles away from home, so bare with me.

Fiance says the truck is acting funny. She said that the truck seemed like it was jumping out of gear or something. I tried to get her to explain more...but. From what I gathered, shes holding constant pressure on the peddle and the truck is jumping. Said it was so bad the other day she drove home with the hazards on. She had my friend drive it (mechanically inclined), he said that it seems like the yoke is bad. But, the problem doesnt seem to fit the solution?

I know its not much to go off of, sorry guys. Any questions you could think to ask and I'll try and get an answer ASAP.

Other info:
1997 F150 4x4 w/5.4

Tranny was rebuilt 4,000 miles ago.

Is she trying to drive it in 4WD on dry pavement? It will buck around corners then. Otherwise it may have developed a misfire. Good luck!

Update:

Talked to my fiance, this is what I was able to gather.

1) Does it sporatically, not just on acceleration or just cruising.
2) Seems to have lost some power (would lean towards a COP)
3) Didn't notice a change in RPMs when it happened/happens.

All things seem to lead to a COP, but theres no CEL on.
Lesson learned today:
Find solution from problems, dont frind problems to fit your solution.

Still didnt get to talk to my buddy, may get more light shed on the situation. She said (he said) that he checked the yoke and it seems looser than its supposed to be. So maybe he just happen to stumple on an upcoming problem that hasn't shown itself yet?

Oh, no, shes not in 4x4, I made sure she knew how to use that before I left. She's a pro.

Plus, when you take off in 4x4 on dry pavement, it doesnt feel like you have lost power

A COP won't set a DTC unless it is a severe misfire. A sporadisc misfire will just cause the bucking. How long has it been since the spark plugs and COP boots have been replaced?

This is why I'm kicking myself right now, I used to be better at this. Too long in this hell hole and I've lost my touch!

COPs....original, have 191,000 miles (hell yeah)

Spark plugs have about 40,000 miles
