Question 2004 6.0L Turbo surge

I have a 2004 F250 6.0L with 30,000 miles which has had the check engine light on for the first year. Every time I take it in they reprogram it and give it back. What I have noticed is the turbo sounds as if it winds up and down while hloding a steady speed. At low speed 20-40 mph a puff of black smoke will discharg from the exaust every time the (turbo sound changes), the smoke is not noticable at high speeds, but the surging sound is still there. There is also a lag when starting from a dead stop if the very light pressure is put on the gas pedal. The truck runs this way at least 50% of the time, when the problem isn't there the truck runs great.

Any help would be great

Update, almost fixed

Just updating related threads.
I picked up my PSD X today, they had it for 3 weeks. They put in a newly redesigned EGR valve and a redesigned turbo. The EGR valve was only replaced in April and the new one was available since June. The turbo looks different, it actually looks better. They also put in the flash that was released late Wednesday and was available Thursday morning. I drove the truck in the city, country and the interstate for hours. It did not smoke or buck. It did however surge like you would believe. It surges in the middle to end of each gear (AT), shifts and surges through the next gear. It won't hold a steady RPM without some surging. They warned me of this when I picked it up and suggest that I drive for a little while it adapts to my driving.
DJM,
My smoking seem to be gone, but the surging is new. Does this sound like what your's does?

No the surging was while holding a steady speed. I just got my truck back three days ago. So far it looks like they fixed the problem, they replaced the EGR valve and put on a new EBP sensor, it's running like a new truck. I would like to thank everyone for thier help. I hope they find your problem I know how it feels.
