oil and coolant mixed!

I have a friend who has a 2001 expedition 5.4L 2WD and he told me that there was a milky substance inside the reservoir where you put the coolant in. My first assumption was that it needs a new radiator. Would an OEM radiator have a heat exchanger? Could it be transmission fluid? Could it also be a blown head gasket? How can I eliminate one of these so that i give a precise diagnostic? Are there any tests i can do to the radiator?

did you check the oil? see if its milky. my money is on head gasket.
