From what I understand the intake air heater should not effect performance but especially when its cold it likes to idle fine then rev uncontrollably for about a second
When the truck is started in the cold, after a minute or so, it will start to idle up. This is normal...... You can stop the high idle by tapping the gas pedal and that will kick it down to a normal idle. The high idle helps your truck warm up when it is just sitting. It is fine and normal.
As for all of the 'codes'. Several could have been there for years and years. Wipe them clean and run the truck for a week or so and check the codes again.
Lets work together from one issue to the next. You just got here. Walk, before you run..... and you will get a lot of good suggestion.<!-- / message -->
This is much more violent than a high idle it is bad enough that when i'm driving down the road on slick surface it will actually break traction when it happens. It runs great when the check engine light is on but horrible when it is off

There is something calling for a quick short increase in ICP. I would disconnect the super chips programmer to see if that stops the "revving". Putting it in stock doesn't really disconnect it.
