'97 5.4l 2V thrown plug - Cal-Van mess up (xpost)

(Anyone notice that Cal-Van drastically increased the price for the 2V repair kit? Like from 265 to 378 in the last week? OMG)
Ok, full admission - I'd installed the Cal-Van insert some time ago, but messed up by not putting JB Weld on it the first time. Eventually the plug was thrown again, marring the threads.
Cleaned out the threads but the actual insert would just spin, even though the tap found and held threads. Put in some JB and re-installed. Made it a few hundred miles and it popped out again.
In reading on the site, found out about the 4-thread issue on these, so now wondering about where I'd put the JB, maybe I needed more at the bottom vs. more at the top end of the insert:
Thought about trying a helicoil - widening the head plug hole to the next larger size, then JB'ing the insert and heli into place. That 4-thread thing makes me wonder if that's feasible?
I have one more unused insert - or, if I might have success cleaning out the one in the pic, could that be re-used? Realizing that if we went with the insert+helicoil, would have to use the one new one I have left.
Any suggestions?
