Locking Gas Door :)
I just got this in the mail yesterday and installed it last night.. The thing looks great (in my opinion, at least)

I paid $50 for it, and it manufactured by "Bully" and it branded by RPB. You can get the one without the RBP star for about $30 on eBay. They make them in black and polished..
I am actually debating painting the center portion to match my body color. Personally I really like the "bolt on" look, I have a RBP replica grill, pocket style flares, a RBP license plate frame, and now this fuel door, which all give me the "bolt on" look.

Google search part number: RBP-144-BK
I got mine from CarID, its was $50 with free shipping and I think on retailmenot.com I found a coupon for something like 5% off..
RBP has this part number listed for fitting a 97-03 F150, but in their fine print it says 99+ Super Duty, so most websites do not show this part fitting a SuperDuty..
