E40D problem

E40D problem
Hi,
I have a 94 E250, 5.8 with e40d. The problem I have is it seems to "slip" or shift between 1-2 or 2-3 gear randomly. Almost like it can't decide which gear it wants to be in and shifts back and forth quickly, or it's slipping can't tell which. It hasn't happened to me but once but the person that drives it said it has done it a lot before.
Just wondering if some one can point me in the right direction. Thanks for any input.
