P0470 is exhaust back pressure sensor circuit malfunction, i believe this was the other code i got till i cleaned my ebps.
P1211 is ICP not controllable, above or below desired, i believe this is from your tuner and is normal with tuned trucks.
I just got back from week long vacation so my brain is kinda out of it but saw nobody else answered so thought i would give it a shot, if nothing else it will bumb this to help you out. Hope you get things worked out.

Ponch is correct on the codes. Two are caused by a plugged backpressure sensor tube or sensor which comes out right by the Water pump, at the front of the motor, and the 1211 is infamous for most trucks with tuners, just means that your HPOP can't maintain the pressure the computer wants. Does not indicate anything broke. If you remove the tune back to stock and still get the 1211, then you need to look into it more.
About the only way to detect a shot injecotr is if the motor is running so poorly that it is detected in the balance test. But for that you will need an actual diagnostics tool such as the AutoEnginuity that can perform the test. There are a few ways you can go with your injectors depending on your ability. You can simply order up a set of rebuilt injectors from a variety of places such as Swamps Diesel and replace them yourself, or you can order rebuild kits from DIY injectors, I wouldn't worry about upgrading injectors either, your HPOP isn't strong enough to max out the stock ones. If you don't mind being down for a bit, Jonothan at Swamps will analyze your stock injectors and give you a full report on them at a reasonable price, just to make certain of the root cause. Etching is caused by a caustic, pitting usually caused by water, and scratches caused by poor filtering. Either or all fo those will contribute to the condition it sounds like you are having, as well as a few other possibilities, still centered on the injector.

Frank- You really should replace them all. There are several reasons for this, but basically if this was caused by fuel, one way or another, than likely all of them are at least slightly damaged. If one injector simply craps out, then that is one thing, but I don't believe that to be the case here.
And HotWheel- If you drive anything like I do, you will contacting me in about 3 months.....

That is hard to say. Chances are that No, you won't mess anything up so long as the injectors are not overfueling or leaking. I would do a blackstone Oil report to make sure that all is well in the bottom end. That will also tell you if you have any fuel in the oil from overfueling. Just make sure you still shut down and startup on diesel to avoid any coking issues.
