Are you thinking the clutch disk itself is flipped?

I believe Ross is saying the clutch disk is flipped, backwards. At first glance, I thought it was correct but now I believe I may have blown my answer. Yes, better pictures are needed.
So far as the OP second picture and question, I don't know what your question is. Normally the yoke fits into a bushing in the tailpiece and the splined shaft fits several inches into the yoke.

Sorry, I've been on the road. Yes I think the disc may be flipped, but hard to tell. Did you take the PP and disc off? Or are they as it was when it was running/driving?

Sometimes "To Flywheel" or "Flywheel side" is stamped on the disc near the hub.

Thanks Kevin! That's exactly the pic we need from F-1.

As far as the yoke, is there a seal iin place on the rear bearing retainer/mount? The yoke needs to go thru the seal. I don't see anything wrong, the yoke just isn't pushed all the way on. If that's as far as it goes, then something is wrong.

So when I put my clutch plate in wrong, I got all kinds of confused until I removed the PP and flipped the plate. Everything got happy real fast. Perhaps you have the same issue.
