86 E350 ambulance, hitch receiver mounting?

86 E350 ambulance, hitch receiver mounting?
I'm looking at this hitches like these two PartsTrain.com - Hard to Find Auto Parts and Truck Parts - Reese Hitch Class III/IV, 2" Box Opening 44027: Advance Auto Parts and it looks like they want to be installed with bolts up from the bottom through the rear section of the frame rails.
Then I'm looking at my frame: Imgur
foreground and background left-to-right are the frame rails. far right is the step-up bumper. The tall bolts and plates that wrap around the frame rails go up to the box/module above and anchor it.
Two problems present themselves.
1) I don't see any vertical holes through the frame rails for the hitch receiver to bolt through.
2) The bumper extends pretty far below the frame rails. I suspect the hitch receiver wants to exist in that same space.
Help?
