95 ford f250 running rough

I have a 1995 ford f250 with a 5.8L engine. It was running fine until the other day when I started it and it was as if I had no power. I shut it off and turned it back on and everything was fine so I just assumed I was in some mud and had spun my tires a little, wishful thinking I guess. I drove it home and shut it off and turned it back on three or four times and it was fine.

Today, when I went out to start it, the same thing happened. It starts, but idels rough and has no power, (I can barely get it to move without dropping the clutch.) I hooked up my code reader and got the following codes:
213 Ignition SPOUT or SAW circuit open or shorted - Ignition Systems
538 System did not receive "goose" test - TESTS


I did some research and found that a bad Elcronic control module could cause this, so I replaced it, but its still doing the same thing. So I hooked up my code reader again and got the following codes:
412 Idle speed system not controlling idle properly (generally idle too low) - ISC
126 MAP or BARO sensor out of range - ">MAP

I figured I was getting close so I replaced the MAP sensor and put in a new fuel filter just in case but its still doing the same thing, it just gives me this code now:
412 Idle speed system not controlling idle properly (generally idle too low) - ISC


I am completely out of ideas and have no idea what do do now. Any help would be much appreciated.

Last night I also replaced the IAC valve and disconnected the exhaust hoping that it was a clogged cat but that didn't help. This morning it was a bit hard to start and get it to idle but once it started up we hooked up our nifty vacuum pressure gauge right in front of the MAP sensor and got 20 inches right off the bat. It held there for a good minute or so but we noticed that it seemed to run better with the MAP unplugged and someone holding their finger over the hose. It seem to run fine for a few minutes and then we hooked the hose back into the MAP sensor and it started running poorly. Thinking we were onto something, we disconnected the vacuum hose on the MAP sensor again, only this time the engine continued to run rough. We hooked the vacuum gauge back up and it read 18 inches, only bumping up to about 21.5 inches for a few seconds after goosing the throttle a tad.



After reconnecting out MAP sensor, we hooked our Snap On code reader up again and when it gave us the "CODE 10" to "goose" the throttle it bogged way down like it was going to die. I didn't think to mention it, but it has pretty much done that every time we did the goose when the machine is telling us to goose it. Anyway, after the check I got the following codes.:

40

411 Idle speed system not controlling idle properly (generally idle too high) - ISC

412 Idle speed system not controlling idle properly (generally idle too low) - ISC

126 MAP or BARO sensor out of range - ">MAP

538 System did not receive "goose" test - TESTS



After this we tried checking the vacuum off the tree that goes into the manifold. There we go 1 inch and it was running rough. Then the commuted revved the engine a little and it bounced up to 20 inches for a few seconds and idled back down and the vacuum bounced around 10 to 12 inches. It also started to idle smoother.



I'm not sure if this will help but right before I came in to report to you, the all knowing expert I unplugged the MAP sensor and noticed that the exhaust was pouring more spoke from where we disconnected the pipes, than it was when the MAP sensor was plugged in. Again, not sure if that tid bit helps, but thought I would trow it in there.

Can anyone please help me on this, I am stumped!!!

It was the fuel pump… for anyone that wants to know v.v
