Fan belt squealing when shifting, no power

2006 v10 f450 84,000 miles

It has new belt idler wheel, new battery, new belt. It consistently squeals when shifting hard or soft on the throttle. Has very weak acceleration, sometimes dies after accelerating real hard. Fan spins freely.
