2008 Taurus X transmission problems

My brother just called me about his Taurus X. When he cranks it you can hear a terrible noise like something slapping around under the engine. He said the noise is loudest under the engine near the tires.
When he puts it in gear, it doesn't want to go. He said it barely moves the vehicle at 2k RPM.
Vehicle has 111k miles.
Any particular failure in these vehicles? Any idea where to start?
