1991 E250 loss of power on hills

Hello
I have a 1991 E250 with a 5.7L engine and a rear gear ratio of 3.55. It goes down the highway fine but on even small hills it pulls right down, even after shifting down, trans checked and serviced. Full tune up, cat replaced, fuel system fully checked and serviced. It has 160,000 Kilometers on her and runs perfect otherwise.
Any idea's would be great

351 or 460 V8?

Would check timing, spark plugs, and I assume it is fuel injected, so make sure the regulator is in good shape. Also check for correct fuel pressure, static and dynamic. How about the fuel filter?
