Still searching for mpgs

I've got an old friend with the same truck from 94. He says he gets 20mpg and he can't put it in OD under 65. I can drive around all day in OD at 40. The difference has to be the rear end, but he's in no shape to be crawling underneath to check.
I'm on a VA pension so I REALLY want better mpg.
Isn't there anybody out there with a higher geared rear?

Have the same exact truck can't destroy it mine has been used as a farm truck and put thro hell am amazing buy for you and I wouldn't sell mine for anything best truck have sitting in my driveway if you ever want to get rid of it contact me plz
