High Pressure Power Steering Line

Over the past 5 months I have had to replace my high pressure powersteering hose. The orginal hose came apart at the pump, went to dealer got replacement hose all good not about month later hose came apart again. Replaces again at dealers cost my time, but today the same hose started leaking again. My question is what causes this to happen!!!!
Next question is there a fix for this problem to keep it from doing it again.

Is your pump making any noises, or wheel hard to turn?

Yes this is because it is low on fluid, and that is because the line is leaking where it go into the fitting the fits into the pump. The teflon sleeve that holds the line in place keeps coming apart.
