Hi everyone, i have a friend with a 4.0L V-6 in a 93XLT explorer with about 120000 miles on it and it has started to make a loud clicking around the passengr side valve cover only when its put into drive or under a load sounds fine in park even if you rev it only under a load, it does it at any speed, if you have any ideas please help i want to help him out before it becomes any bigger of a problem as he doesent have alot of money for this kinda thing. THANKS
He might try flushing the engine, and getting some fresh oil and filter. Here's a flush I just did on one of my vehicles that has 100k miles on it(not an 4.0). I drained the existing oil but left the filter in, and put in fresh and cheap 5W30 oil (3 quarts). I then added about a quart of ATF and let it idle for around 15 minutes, reving it up now and then (this should help clean out some deposits). Then I drained the oil again, and added an K&N High Performance Oil Filter ($10), and Mobile 1 5W30 (You probably want to use 10W30). Last I replaced the air filter. The car is now quiet as can be and has noticeably more power.

Could be a worn push-rod. I have read one article that indicates some 4.0 litre's may suffer from high wear at the pushrod/rocker contact area.
my 2.9 b2 does this aswell. not mention the truck is shot. personnaly ive herd that it could be pushrods cam lifters bad valve adjustment ect. i dont know what causing it and the truck goes for sale soon (after i swap the trans in my tempo) and it wont be my prob then. although im curius to know what it is.
I've got a 92 exploder that does something similar to that. Mine makes a muffled knock under load, I think its piston slap. If yours is the same it could be piston slap. If it was in the valvetrain, it would do it all the time. I also had an exhaust manifold gasket blow out on mine, that made exactly the sound you describe, only under load. Something else to think about would be a head gasket. Hope I didn't confuse you too much. Good luck!
Rule out the Head Gasket by checking the oil hot. Is it milky? Nope, didn't think so. Exhaust manifold Gasket? Okay, probably not, but start the car and check anyway. It will be a ticking noise and you'll feel warm air come out wherever the crack is. Valve train. Now we're getting somewhere. Sorry to say it but valve train will make more noise (when cold) under a load, and when warmed up it may not make any noise at an idle. So, use a stethescope and listed to the passenger side valve cover. I have had cars that did this and changing the lifters (simple project) solved it. Cheap and easy. Don't go ripping your car apart trying to replace every little thing. Get the right tools and make an educated guess.
