You need to fully identify the subject vehicle including year, engine, transmission, drivetrain, and any significant modifications including tuners, etc.
Otherwise, anyone interested in looking up the fault code has no idea where to look.

I'd blame it on the Hypertech. Call them.
That code was only used by Ford on some of their diesels, therefore, I think the tune is wrong.
