Had a home made unit that I made from a honda civic aluminum strut brace. Cut it in half and created a pivot, blah, blah, blah.
I have an Advance Adapters cable set-up now.
I made my own twin sticks using two stock 78 np205 shifters. Each shifter has to have its spacer cut down till both shifters fit on the one mounting bolt. I then used I think, 3/8 metal rod to make the links from shifter to shift rails. This method will require some trimming of the transmission tunnel to fit, but it works great.
I am now in the process of trying to peice two boots together to fit the shifters, if this fails, i'll try the dual shifter boot thats available for the jeeps.
Its pretty cool putting the front axle in gear (rear in neutral) and making sharp turns in the mud to line up with on another hole. But be warned, it might be pretty hard on anything less than a D60, im using rockwells.
Hope this helps, Dave.
