Everything is a compromise and it depends on how you use your truck. If you haul heavy loads on city streets, regearing the diffs for more low end grunt might help. If you drive on the highway with minimal hills, a programmable chip may have some tweaks that will give you a bit higher mpg. Keep in mind that if you optimize for one type of driving, you may be penalized for another. That said, you're only going to gain a couple of mpg. There are no magic bullets.

I saw 19.5 MPG once in my truck. Changing the plugs and coils was the last thing I did before I got mid 15s- mid 19s. However, the latest update to my sniper special forces software was advertised as giving 15% more mileage. I also had put in a factory tech valve body the same day as the tune update.
Combo of 10 miles back and forth to work and 20 miles to the shooting range weekly have given me about 13mpg. Up from 9-10. The 19 was on the interstate cruising at 62mph.

I have a 97 Eddie Bauer that was giving 12-13 mpg's on street and 15-16 mpg's on highway. I upgraded the exhaust with a flowmaster at first and gave me about 1 more mpg to each and more kick. A year later I switched my stock air filter into a stock replacement K&N filter(not cold intake because of Cali regs) which gave it more kick, switched to premium fully senthetic oil, use 91 octane and keep it real light on the pedal. I kid you not, my readings are 15-17 mpg's on street and 18-20 on the highway. Sometimes 21 on a flat longdrive at 71 mph. And I'm still within Cali regs. That's the same mileage on the "new and improved" engines.
