Code #1405 on 01 expy 5.4 ?

Code #1405 on 01 expy 5.4 ?
First off I'd like to say hello to everyone. I just signed up today but have been lurking for sometime now. I feel pretty guilty about my first post being a question, but I'm in a bind. Anyways my wife gives me a call a couple days ago that her check engine light came on and is staying on. When I read the codes with my SCMT, a code P1405 appeared. The code is discribed as:
P1405 - dpf egr sensor- Look for signs of upstream hose off or water or icing plugged hose. I need A little guidience in where to locate this hose. My code book lists a p1405 as the upstream hose and a p1406 is the downstream hose. How will I know the difference? Has this been a problem for anyone else here, and how did you get it resolved? I can clear the code and the check engine light will go out but will return within the day. Any help is greatly appreciated?
