Rattle around 300-400/3200+ rpm's. Thinking DMFW, but disapears when it warms up?

Rattle around 300-400/3200+ rpm's. Thinking DMFW, but disapears when it warms up?

A truck I am looking at has a "rattle" (for lack of a better term) around 300-400 rpm with the clutch ingaged or disengaged. It also has this noise past 3200 rpm. It is a fairly slight noise, but still noticable. It disapears when the everything gets warmed up(bearing?). From reading previous post, it sounds like the DMFW, but it going away when it warms up doesn't seem to fit. There is also trouble getting it into reverse, all other gears are smooth. Can someone confirm my suspicion that this could very well be an exspensive fix down the road? 221000 miles, leaves me wondering

P.S. He said the clutch was changed 30000 miles ago, and he had a mechanic check for the noise. he was stumped. he also says it has always had that noise(he bought it 110000 miles ago).
