Last year a friend of mine has a shop and had the same thing happen with a lifesquad they were working on. They changed the oil cooler, and was fine for a 100 miles or so, then it would start running hot. Oil cooler was plugging up. Turns out some dip**** at the firehouse poured some radiator stop leak in it to "fix" the notorious radiator leaks that these trucks are known for. In a nutshell, they changed the oil cooler 5 times before it was cured. Pulling the hoses off, flushing the radiator got some of that schit out, but the oil coolers caught most of it.

My buddy was having the same issue as you, he installed one of the external oil coolers and it helped his temps when towing but he can't idle his truck for too long now or it will overheat
