Thanks Pawpaw for your support. I think this problem should be resolved for good and hopefully it will help someone else in the future. Great forum here and I have learned a lot about my old truck...
One last question for you, Pawpaw. How hard to replace the timing belt on my truck. I see some tiny hairline cracks on the belt's surface (from the top). The last time it was replaced was about 200K miles ago and it was done by a shop. He used Gates. I checked few clips from Youtube, it doesn't look too bad. Just wanted to see your suggestion. I know I need a special tool to remove the tension belt. My main concern is to remove the big bolt at the bottom of the engine (near the crank sensor). I remember the guy at the shop told me it took him long time to get that bolt loose.

BUMP:
I see I forgot to answer your timing belt replacement question. I've never owned a 4banger Ranger, so never had to replace a timing belt on one, but several forum members have.
With 200K on this belt it's lived Two lifetimes & with you seeing cracks on the Outside, its Way past time to be replaced, so if you haven't done it since mid March 2018, do it without further delay as winter is upon us & when its cold & stiff its more likely to break in cold weather.
Since its been in there 200K I'd change the idler & tensioner pulleys too, along with the water pump while your in there.
