1995 F-150 rough idle

1995 F150 5.0 213000 miles

Been driving this truck daily, but not many miles, since March and its been running good until about 2 weeks ago. Started up put in reverse and idled really rough/surging, started running good once I got going down the road at 50-60 mph. No CEL, no codes, Been doing this most days this past week, idles really rough, stalls sometimes. Runs worse when transmission is in drive or reverse at low speed, OK if I leave it running in park. Seems to be related to warm weather, as its always runs good in the morning.

It was doing this about a year ago too, I wasn't driving it every day at that time and took the truck off the road for a while since it had a lot of other little problems, most of which I've fixed. In the last year I have replaced ECT sensor, O2 sensor, TPS, thermostat, entire exhaust system, all vacuum lines in engine bay, fuel filter, spark plugs and wires, IAC valve, and some other things.
I've only driven it about 3000 miles in that time.

What should I be looking at?

No codes? Did you mean a system pass code of "111"?

Does the CEL illuminate when you turn the key to "START"?
Does the CEL illuminate when cranking the engine?

I realized I've been reading the codes wrong. I can only get the light to flash by grounding the single self-test input STI plug, I was trying to get it by grounding pin 6 but that doesn't work. I got:

113: High voltage air change temperature sensor (ACT)
117: Low voltage Engine coolant temperature sensor (ECT)
122: Low voltage throttle position sensor (TPS)
123: High voltage throttle position sensor (TPS)
157: Low voltage mass air flow sensor (MAF)
211: PIP circuit fault
212: Tach, circuit, SPOUT grounded. SPOUT circuit, open circuit
332: EGR valve not opening
542: Fuel pump (FP) circuit failure
659: High vehicle speed in park
668: Transmission range sensor circuit above maximum voltage

I looked at the TPS, removed it, I think I may have installed it wrong the first time, put it back on, also checked the EGR valve with a vacuum pump and it looked fine. Cleared codes by leaving battery cable undone and drove it the next day. The CEL always comes on soon after it starts running now. Only 3 of the codes came back: 332, 542, 668.

Seemed to drive good for a few days, then today it was back to running rough. I tried checking again for any additional codes and couldn't get the light to flash, it just goes off after a few seconds if I have the STI wire grounded. If I don't have the wire there the CEL stays on with the key on.

I guess the next things for me to look at are the EGR solenoid, and transmission range sensor. Still a little confused about reading the codes too. Shouldn't the light start flashing every time I turn the key, if the STI plug is grounded?
