Very rough idle with video. '95 7.3

I just bought a '95 F-350 a few weeks ago in it's current running condition. It has 180,000 miles on it with a fresh oil change and new HPOP. Previous owner didn't know what was wrong with it. This is my first powerstroke, and I've learned a lot about them in the past few weeks but I'm not sure where to start with this one.

It's starts very hard in cold weather. Lots of white smoke, and a lot of cranking. The past few days that I have started it, it has been 60-70 degrees Fahrenheit. In the warmer weather it takes about 5 seconds to start with a puff of white smoke. Regardless of the outside temperature, when it finally fires up, it runs very smooth for the first 3-5 minutes. It will idle and rev with no signs of any problems. Out of nowhere it will the idle will spike and start running extremely rough. It partially smooths out when you rev it, and if you shut it off and start it back up, it will run smooth again for about 5 seconds. I have driven it across town with it running like this and not once did it die. I checked the codes with my edge programmer and it comes up with a P1211. I've run the truck with the ICP unplugged it didn't change at all. I have unplugged all 4 UVCH connectors one at a time with no noticeable change. Has anyone seen these symptoms before? I'm not sure where to look next.

to look up code

Looks like IPC or IPR issue unless you have air in system still which can take a good 60 miles of driving to get out.

remove IPC connector and see if it does the same and to see if oil is in plug that means its leaking too

I mentioned that in my original post. Unplugging the sensor had no change.

Hpop was swapped by the previous owner. I guess I could look at the IPR to see if it was replaced or leaking.

Loose uvc conectors or burnt pins would be my first guess. Remove the valve covers and take a peak and look for the obious.
