Hey guys I have a 06 f350 with 5.4 140k miles. Recently it has been shaking bad on idle but only in gear and it's hard to start when cold. The exhaust is moist at all times and the fluids never leakWhen you floor it it hesitates feels like it's overly rich but if you let off a little it stars to pull again. Also while cruising at 1500-1700 rpms in any gear it will surge and skip. Only time it will miss and the CEL comes on is over 4000 rpms for a random misfire. Just put new coils in new fuel pump and filter. There has always been a cat code but it never effected the drivability.

Could it be injectors?

I'd start by looking at the cats. A clogged cat will cause a lot of odd issues.

Plugs? When did you have the plugs done? I was having similar issues at 106k in my 05 with 5.4L. Rumor had been that the plugs were changed one in my motor (turned out to be original plugs and worn waaaay down). Would idle fine in neutral. moderate to heavy throttle would get a random misfire and at idle with mild load (in gear) it would stumble. Lots of hesitation.
I was dreading tackling the plugs but it went way better than expected. Took a while to get them all done. All hesitation gone. smoothed out idle, no cel. No more surging.
