I think it's off a 55 ford, probably modified to make it work.

I'm thinking it's a 55 grille that has been customized with the quad headlights, but I am not 100% sure. I've never seen one like it before. I don't think quad headlight were on cars or trucks until 58.

Its a 55 grill that he modified to fit...
