Did a little searching and came up with this thread: CrewCab Headliner Removal with Pics It may not be the 'right way of doing it. Also a little further down, some one mentions just flipping it in the cab and applying the new material, with out taking it fully out.
Just thought i'd do some research since the same question has been plaguing me, as I don't believe I'll have the funds to do the full interior yet, and don't want to wait on the interior to be able to drive the truck.

Scored a free OEM crew cab carpet fe2om a guy scrapping his truck. Needs to be cleaned and cut a hole for the transfer case shifter. Also scored NOS dash bottom panel and both side A-pillar mouldings.
Dirty!
Test run to see if it is worth it to leep it. I cleaned the top 1/3 and it came out looking awesome!

purple power i splashed on out of the bottle and scrubbed the hell out of it. Folex you spray on, agitate with your fingers, and blot out stain with paper towel. Or a carpet extractor if you have one
