E40D, Heavy Tow Load, Won't accelerate forward after stopping on hill.

I have a 1996 F350 P/S with about 300,000. I just purchased a large 5th wheel and have no problems towing normally. I have problems stopping, but that's a different question.
I was in Seattle and stopped for a light on a steep hill. When the light turned green I stepped on the accelerator, engine revved up but the truck didn't move. After a second it began to creep forward just a little. Accelerator was floored. It moved forward very slowly and I made it to level ground.
On small hills and level ground it accelerates like I'm towing a big trailer, but I have never had it not move before. I think if I had stopped and put it in low-range it would have made it. Is it too much weight? Worn transmission? What causes that?
