What's the odd cutout in the gravel pan for? A winch?

Yup, me too. Prolly was a fire truck with the dual spotlights, West coast mirrors, and red grille..... And it is a Big Job.

Does the mileage matter if it's just sheet metal and no drivetrain...?

Those old Firetrucks were well kept & usually low mileage. Just great cabs to start with because of their upkeep.
