To the best of my knowledge you'll need 5 pieces altogether to make an original mounting system.
C5TZ-1443-A - Spare Wheel Carrier Channel. (assumes you did not have 19.5 inch wheels)
C5TZ-1408-C - Spare Wheel Carrier Support Bolt. Overall length including loop 12 9/16"
353735-S100 - Bolt-Channel to Frame-left (drivers) side / 1/2" -13 x 10 1/2" long
384581-S100 - Clip and Blind Nut - retains bolts to frame (2 each of these)

The only pieces I can find still available NOS is the last one - the clip and blind nut. They probably are going to be a hex nut instead of the original style square nut, but they will be correct size and function properly.
Some people use carriers from other trucks, like S10. Search this forum for spare tire carrier and read a few.
