The steering wheel in my 79 F-350 4x4 is supposd to be centered going straight down the road like 3 and 9 oclock but it varies between 3 and 9 to as much as 1 and 7 oclock. Is this the steering box or should I be looking for something else worn out ?

have someone rock the steering wheel back and forth while you look at the steering components. That will show you where the slop is and you can go from there. I'm betting tie rod ends are shot
Could be a deteriorated rag joint/steering shaft...
Could be a sloppy or loose pitman arm....
Could be worn tie rods....
Could be a worn drag link...
Could be a loose/worn adjusting sleeve....
.....Prolly why it's called a steering "system", right?
Have a helper provide input on the steering wheel while you observe the steering components... everything should move in unison. Start at the steering shaft input to the steering gear while observing the pitman arm... and then the shin bone is connected to the leg bone, the leg bone is connected to the hip bone, et cetera, et cetera, et cetera...
4x4 F350 does not have a drag link only F150...got ya lol
All the above and insp the steering shaft u joint, and add up worn ball joint or king pins (D60 front axle), wheel bearing play...and all the rest of the worn components it all adds up to looking like a drunk while driving down the road.
Have you tried to adjust the steering box play? If you have and it is shot, Redhead time. Also insp the adjustable steering link ends. That is the short piece that goes from the pitman arm to the dvrs side spindle.
