1995 ford aerostar misfire

The van misfires pretty bad when cold it gets smoother when its warmed up but still misfires. Has new wires , plug and mass air flow sensor. I recorded the check engine light while the vehicle was running and this is what came up. Also the other day after driving for about three miles the van would not start it cranked but wouldn't start and this morning it started fine but again , is miss firing pretty bad. I cant seem to figure out the code sequence its giving me.

Check the simple things first; look for a vacuum leak, especially around the vacuum tree on the back of the intake manifold on the driver's side
